Complex Patient Management Group (CPMG)

The aims of the team are to:

· Support local GPs in the management of their most complex patients. To provide advice, support and education on how to do this in the community
· Advise on strategies in both medical and social care provision to prevent unplanned hospital admissions and promote wellness
· Advise on services to promote health and wellbeing via the health, social care and voluntary sectors for patients, families and careers.
· Provide your GP with a cohesive care plan following your assessment and review by the CPMG team.
· Once stable, the CPMG team will discharge you back to your GP with an open door to re-referral if required.
